ciu new officeThe Department of Social Welfare and Development – National Capital Region (DSWD-NCR) would like to inform the public that its Crisis Intervention Unit (CIU) that processes assistance to individuals in crisis situation such as medical, transportation, burial, and educational, among others, has transferred to its new office located in Gastambide Street beside Mary Chiles Hospital, Sampaloc, Manila.

 

“With our new office, we can better serve our clients and improve delivery of our services to those in need. We have dedicated social workers and staff who will be assisting them throughout the process.” DSWD-NCR Regional Director Ma. Alicia S. Bonoan told.

 
She also reiterated the requirements needed, as follows:
1. Medical Assistance

a. Clinical Abstract/ Medical Certificate with signature and license number of the attending physician (issued not later than three (3) months)
b. Hospital bill (for payment of hospital,) or Prescription (for medical medicines) or Laboratory Requests (for procedure)
c. Barangay Certificate/ Indigency and any Valid ID of the client

2. Burial Assistance

a. Funeral contract
b. Death Certificate of the deceased
c. Barangay Certificate/ Indigency and any valid ID of the client
d. Permit to transfer/ Health Permit (for transfer of cadaver)
e. Referral Letter (if applicable)

3. Transportation Assistance

a. Barangay Certificate/ Indigency and any valid ID of the client
b. Police Blotter (for victims of pick pocket, illegal recruitment, etc.)
c. Referral Letter (if applicable)
4. Educational Assistance

a. Enrollment Assessment Form or Certificate of Enrollment or Registration
b. School ID of the student/ beneficiary
c. Barangay Certificate/ Indigency and any valid ID of the client
d. Referral Letter (if applicable)

 
Bonoan further reiterated clients to ensure that proper documents are available for faster processing of their request. They were also reminded to beware of the fixers and to transact only to authorized staff of DSWD-NCR. ###
